Please print this form on 8-1/2 x 14 inch (legal) size paper.
Carefully fill in all fields and deliver to the Department of Marine & Ports Services, Boats and Moorings Office (old Paget Post Office)
for processing by the Bermuda Police Service.
Please be advised that as of September 2011 there is a $100.00 fee for all vetting work undertaken by the Police Service.
There is a pre-existing fee of $80.00 for pass processing and printing work undertaken by the Dept. of Marine & Ports.
Any cheques should be made payable to the Accountant General.
Once your application has been successfully vetted and printed, you will be contacted regarding the collection of your pass.
Applicants for HM Customs Inland Freight Facility passes should utilise the same
application form for the purposes of Customs vetting and issuance of a LIC pass (simply tick the appropriate field).
